    Send the completed order form to the Parish of Saint Theodore, Port Talbot, along with your cheque or postal order payment and we'll do the rest.

  2. Fill out.
    Fill out the order form and give details of your donation. Fill out the Gift Aid Declaration if you are a taxpayer and you wish for us to claim back tax on any donations.
